This would add another big element of realism to the game imo. I assume it could be done with animation but would it be possible with physics? Thoughts?

I had also thought about why this isn't built-in. It is definitely possible, yes, but it might be necessary to edit the mesh or animation bones to support it. I don't know as I haven't been looking at them.

It would be a neat addition also for larger insertions I'd like to see belly bulge also.

For belly-bulge, I vote for the ability to fist the girls. Both ways. This is in tune with the domination parts of the game ...

Pocket bulge with Cash is what i like? But back to the question Throat Bulge is my preferance:
