I encountered the same in the Love Party mod which has been "incorporated" into this one. Guess some bugs got "incorporated" too.

I think I may have found the issue, Monica is a little bit too high so Saiko cant reach and wouldnt be able to fuck her either as she would need to bend her knees. That's just the state of physics in this version of the game. It's especially evident when the girls are in puppet mode. There is no real solution. Hopefully 4.0 will have better boundary checking to prevent things from going crazy.
